一位叙利亚裔美国医生的传奇
最近阅读了《纽约邮报》记者 Susannah Cahalan 的自传《BRAIN ON FIRE-My Month of Madness》(脑火-我发疯的那个月)，也看了根据自传改编的同名电影，电影很生硬，远不及读自传能感受到的震撼。自传讲叙了作者得了一种叫抗NMDA受体脑炎的罕见病，并记录了从发病到康复的全过程。作者能完全康复是个奇迹，而这个奇迹的发生完全得益于她在对的时间碰到了对的医生，而这个对的医生就是来自叙利亚的纽约大学神经科医生Najjar(图1, Najjar医生与作者)。
Najjar医生出生于叙利亚的富裕商人家庭，父亲从小就将他送到私立天主教会学校接受最好的教育，但他在学校的成绩极差，几乎门门功课不及格，学校校长在他十岁那年跟他父亲说"你的孩子我们是爱莫能助了，要知道上学不一定适合每个小孩，你孩子去学门手艺应该是他最好的出路"。父亲虽然很生气，但他知道教育对小孩太重要了，虽然对自己孩子没有太大的期望了，但他还是坚持把Najjar送到公立学校去学习。在公立学校的第一年，一位老师注意到了这孩子，时时关心他，在课堂上也常常表扬他，慢慢地让Najjar恢复了信心。第一年学期结束时，Najjar带回家的已经是各科全A(straight A)的成绩单了。父亲看了Najjar带回家的成绩单很生气，说"你在欺骗"，举手就要揍他。第二天父母去见学校老师，跟老师说"我儿子不会有这样好的成绩，他一定在欺骗"，老师回答说"我可以向你保证，你儿子没有欺骗"。父亲接着说"你们这是什么学校啊？像我儿子这样的小孩都能有这么好的成绩？", 老师停顿了一下, 接着说"你想没想过你其实有一个很聪明的孩子，你应该相信他"。Najjar 命运从此改变, 他以学校最好成绩之一毕业于大马士革大学医学院，随后移民美国，经过多年的努力，成为了《纽约时报》评选出的全美著名的神经科医生，他也是神经病理及癫痫病专家。2020年他被评为 Ellis Island Medal of Honor(埃利斯岛荣誉勋章) 的受奖人。
Dr Najjar除了医术好，对病人极富同情心。他受邀参加作者Susannah的治疗团队时，作者及家人己处在绝望的时候，诊疗费己花费了上百万，但所有检查，检测都正常，后来的脊髓液检查也只是白细胞稍高(20个，正常是0到5个)，由于病人(作者)除了几次癫痫发作外，主要表现为各种严重的精神病症状，他的治疗医生正考虑将她作为精神病人送到精神病房治疗，如果没有Dr Najjar 的参与，病人可能的结果是，在精神病院里病情恶化，最终走向死亡。Dr Najjar重新仔细进行了问诊及体检，并且不放过每个细节，检查中在Susannah只能画出1-12数字都在右边的时钟时(图2)，让作者的病情出现了曙光，病人的这个简单时钟图让Najjar医生终于能够确定Susannah的病变是器质性的，位于右侧大脑，排除了纯精神病的可能，随后Dr Najjar又用排除法及根据脊髓液的白细胞升高(第二次检查已达80个)现象等确定病人患有自身免疫引起的大脑炎，通过送检确定是一种最新发现，罕见的抗NMDA受体脑炎，Susannah是全世界这种脑炎第217位确诊病人。Susannah一旦确诊后，Dr Najjar 立刻给与病人对症治疗，病人病情明显好转，迅速踏上了康复之路。让Susannah感慨的是，她前后经历了多个医生，从来没有一个医生像Dr Najjar那样认真问病史及做常规的体检，她如果一开始就遇到Dr Najjar, 就不会经历那么大的痛苦及生与死的搏斗。Dr Najjar 常常跟病人说，我会尽最大努力帮助你，请不要放弃生命中的任何希望，我自己的经历就是不放弃希望的最好例证。
Dr Najjar 治愈作者Susannah罕见病的经历通过作者Susannah的自传报道传回了叙利亚，叙利亚各大报刊都给予了转载，这让其父亲觉得是一生中最荣耀及骄傲的事，这种荣耀及骄傲远高于他自己曾建设了首都大马士革很多有名建筑和大马士革机场带来的荣耀和骄傲。是呀，他可能从没想到过自己的小混混儿子能有今天，成为了受人尊敬的著名美国医学专家。

They are good. Only problem is the AF1 logo being too high up on one of the shoes and going under the stitching, and the stitching has some ties at the end and some loose threads. You can’t see it from afar though. After time, the shoes did start to wear with the threads getting frayed and some of the bits that rub together losing their colour.
